Senior Case Manager - care planning (remote)

Ironwood Beacon Community Partners Group is looking for a Senior Case Manager. The job centres on care planning for volunteer shifts, and it would suit someone who likes steady, practical work. You would join a large organisation of about 732 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for volunteer shifts. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with quarterly visits in Oklahoma City. • Applications are reviewed on a rolling basis.
