Senior Case Manager - care planning (remote)

We are growing our team at Harborlight Nest Community Partners and adding a Senior Case Manager whose main responsibility will be care planning. You would join a small organisation of about 33 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for volunteer shifts. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Kansas City. • We will tell you what to expect at each step of the hiring process.
