Senior Case Manager - care planning (remote)

Ironwood Meadow Community Partners is looking for a Senior Case Manager. The job centres on care planning for client intake, and it would suit someone who likes steady, practical work. You would join a small organisation of about 55 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client intake. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • Fully remote, with an occasional day on site in Fort Worth. • We reply to every applicant.
