Senior Case Manager - care planning (remote)

Bluepeak Stone Care Services Associates supports families and individuals in Tampa with personal care and coordination, and we need a Senior Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client records. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Tampa. • Applications are reviewed on a rolling basis.
