Senior Case Manager - care planning (remote)

Bluepeak Hollow Health Group runs clinics and patient services around Raleigh. We are hiring a Senior Case Manager to take on care planning. We are busy and kind, and we look after each other. What you will do • Help with applications and appointments for patient education. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Fully remote, with quarterly visits in Raleigh. • We will tell you what to expect at each step of the hiring process.
