Senior Case Manager - care planning (remote)

We are growing our team at Ironwood Stone Care Services and adding a Senior Case Manager whose main responsibility will be care planning. You would join a small organisation of about 32 people, working mostly from home. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Boise. • Applications are reviewed on a rolling basis.
