Senior Case Manager - care planning (remote)

Lumen Basin Health is a healthcare provider in Cleveland with clinics and support teams. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • Fully remote, with a monthly team day in Cleveland. • Applications are reviewed on a rolling basis.
