Senior Case Manager - care planning (remote)

We are growing our team at Bluepeak Fox Housing and adding a Senior Case Manager whose main responsibility will be care planning. We value patience and follow-through. What you will do • Help with applications and appointments for waiting lists. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Cleveland. • Applications are reviewed on a rolling basis.
