Senior Case Manager - care planning (remote)

Foxglove Bright Learning Group is an education provider in Fort Worth serving students and families. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for enrolment. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with enrolment. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • Remote-friendly, with a monthly team day in Fort Worth. • We will tell you what to expect at each step of the hiring process.
