Senior Case Manager - care planning (remote)

Harborlight Summit Learning Group is looking for a Senior Case Manager. The job centres on care planning for attendance records, and it would suit someone who likes steady, practical work. We follow the school calendar and we like people who like learning. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for attendance records. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with attendance records. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Philadelphia. • We reply to every applicant.
