Senior Case Manager - care planning (remote)

Rivermark Rowan Care Services is a social-care provider serving older adults and people with disabilities around Seattle, and we need a Senior Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for visit scheduling. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Seattle. • We will tell you what to expect at each step of the hiring process.
