Senior Case Manager - care planning (remote)

Fernwood Thistle Learning Group is looking for a Senior Case Manager. The job centres on care planning for family communication, and it would suit someone who likes steady, practical work. You would join a small organisation of about 45 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Full-time, Monday to Friday. • Fully remote, with quarterly visits in Pittsburgh. • We reply to every applicant.
