Senior Case Manager - care planning (remote)

Stonegate Drift Health is a healthcare provider in Seattle with clinics and support teams, and we need a Senior Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for insurance authorisations. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Contract for four months, with the possibility of becoming permanent. • Fully remote, with an occasional day on site in Seattle. • We will tell you what to expect at each step of the hiring process.
