Senior Case Manager - care planning (remote)

Lumen Beacon Housing is a nonprofit housing provider in Rochester, and we need a Senior Case Manager to lead our care planning. Housing is our mission and residents are our neighbours. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for resident services. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with resident services. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Rochester. • We will tell you what to expect at each step of the hiring process.
