Senior Case Manager - care planning (remote)

Harborlight Ashton Health Partners runs clinics and patient services around Tucson. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient education. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with quarterly visits in Tucson. • We will tell you what to expect at each step of the hiring process.
