Senior Case Manager - care planning (remote)

Greenridge Loom Early Learning runs childcare centres in Rochester caring for infants, toddlers and preschoolers. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for classroom supplies. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with classroom supplies. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Fully remote, with quarterly visits in Rochester. • We reply to every applicant.
