Senior Case Manager - care planning (remote)

Cedar Glen Health Group provides primary and specialty care in Cleveland, and we need a Senior Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for clinic supplies. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Cleveland. • Applications are reviewed on a rolling basis.
