Senior Case Manager - care planning (remote)

Summit Maple Community Partners is a Anchorage charity working with residents on basic needs and opportunity. We are hiring a Senior Case Manager to take on care planning. You would join a small organisation of about 30 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for food distribution. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Contract for nine months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Anchorage. • We will tell you what to expect at each step of the hiring process.
