Senior Case Manager - care planning (remote)

Foxglove Clear Care Services Group supports families and individuals in Toledo with personal care and coordination. We are hiring a Senior Case Manager to take on care planning. The work is personal and so is the culture. What you will do • Help with applications and appointments for care plans. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Toledo. • Applications are reviewed on a rolling basis.
