Senior Case Manager - care planning (remote)

Nova Kestrel Community Partners runs community programmes across Tucson. We are hiring a Senior Case Manager to take on care planning. Mission matters here, and so does looking after staff. What you will do • Connect clients with partner agencies. • Help with applications and appointments for donor records. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with donor records. Working arrangement • Contract for four months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Tucson. • We will tell you what to expect at each step of the hiring process.
