Senior Case Manager - care planning (remote)

Amberfield Sable Housing manages affordable housing communities across Austin, and we need a Senior Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for maintenance stores. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • Fully remote, with quarterly visits in Austin. • We reply to every applicant.
