Senior Case Manager - care planning (remote)

Nova Health runs clinics and patient services around Orlando. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient scheduling. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Full-time, Monday to Friday. • Fully remote, with an occasional day on site in Orlando. • We will tell you what to expect at each step of the hiring process.
