Senior Case Manager - care planning (remote)

Bright Field Housing manages affordable housing communities across San Jose. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for waiting lists. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with waiting lists. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in San Jose. • We will tell you what to expect at each step of the hiring process.
