Senior Case Manager - care planning (remote)

We are growing our team at Rivermark Junction Housing and adding a Senior Case Manager whose main responsibility will be care planning. We value patience and follow-through. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for maintenance stores. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • Fully remote, with an occasional day on site in Memphis. • We will tell you what to expect at each step of the hiring process.
