Senior Case Manager - care planning (remote)

Stonegate Oak Care Services provides in-home and community support in Minneapolis. We are hiring a Senior Case Manager to take on care planning. We train and promote from within. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for visit scheduling. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in Minneapolis. • We will tell you what to expect at each step of the hiring process.
