Senior Case Manager - care planning (remote)

Rivermark Yarrow Civic Services supports public agencies in Indianapolis with budgeting, records and programmes. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for budget submissions. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with budget submissions. Working arrangement • Part-time, about 28 hours a week, spread across three days. • Fully remote, with an occasional day on site in Indianapolis. • Applications are reviewed on a rolling basis.
