Senior Case Manager - care planning (remote)

Stonegate Iron Property Management is a Jackson property manager with leasing, maintenance and resident services. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for resident events. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with resident events. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Fully remote, with quarterly visits in Jackson. • We will tell you what to expect at each step of the hiring process.
