Senior Case Manager - care planning (remote)

We are growing our team at Fernwood North Early Learning Partners and adding a Senior Case Manager whose main responsibility will be care planning. You would join a large organisation of about 580 people, working mostly from home. What you will do • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Grand Rapids. • Applications are reviewed on a rolling basis.
