Senior Case Manager - care planning (remote)

Ironwood Stone Care Services supports families and individuals in Colorado Springs with personal care and coordination. We are hiring a Senior Case Manager to take on care planning. You would join a small organisation of about 45 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Colorado Springs. • We reply to every applicant.
