Senior Case Manager - care planning (remote)

Ironwood Linden Health is looking for a Senior Case Manager. The job centres on care planning for patient scheduling, and it would suit someone who likes steady, practical work. You would join a small organisation of about 93 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for patient scheduling. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Remote-friendly, with quarterly visits in Jacksonville. • Applications are reviewed on a rolling basis.
