Senior Case Manager - care planning (remote)

Summit Pike Property Management looks after rental homes across the Jacksonville area. We are hiring a Senior Case Manager to take on care planning. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Help with applications and appointments for lease renewals. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with lease renewals. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Jacksonville. • We reply to every applicant.
