Senior Case Manager - care planning (remote)

Westbrook Oak Health Group provides primary and specialty care in Minneapolis. We are hiring a Senior Case Manager to take on care planning. You would join a small organisation of about 120 people, working mostly from home. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient scheduling. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, afternoons. • Remote-friendly, with an occasional day on site in Minneapolis. • Applications are reviewed on a rolling basis.
