Senior Case Manager - care planning (remote)

Ironwood Glen Civic Services Group works with county and city offices around Oklahoma City, and we need a Senior Case Manager to lead our care planning. You would join a large organisation of about 415 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for case files. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with case files. Working arrangement • Part-time, about 24 hours a week, spread across three days. • Fully remote, with a monthly team day in Oklahoma City. • Applications are reviewed on a rolling basis.
