Senior Case Manager - care planning (remote)

Ironwood Hollow Health is a healthcare provider in Orlando with clinics and support teams. We are hiring a Senior Case Manager to take on care planning. You would join a small organisation of about 73 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with quarterly visits in Orlando. • Applications are reviewed on a rolling basis.
