Senior Case Manager - care planning (remote)

Stonegate Beacon Community Partners runs community programmes across Richmond. We are hiring a Senior Case Manager to take on care planning. Mission matters here, and so does looking after staff. What you will do • Connect clients with partner agencies. • Help with applications and appointments for volunteer shifts. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts. Working arrangement • Part-time, about 28 hours a week, with flexible days. • Remote-friendly, with a monthly team day in Richmond. • We will tell you what to expect at each step of the hiring process.
