Senior Case Manager - care planning (remote)

Bluepeak Rowan Care Services is a social-care provider serving older adults and people with disabilities around Fort Worth, and we need a Senior Case Manager to lead our care planning. You would join a small organisation of about 142 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for care plans. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Fort Worth. • Applications are reviewed on a rolling basis.
