Senior Case Manager - care planning (remote)

Lumen Amber Property Management looks after rental homes across the Tulsa area. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for lease renewals. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with lease renewals. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Fully remote, with quarterly visits in Tulsa. • Applications are reviewed on a rolling basis.
