Senior Case Manager - care planning (remote)

Lumen Willow Property Management manages apartment communities around Atlanta. We are hiring a Senior Case Manager to take on care planning. You would join a small organisation of about 81 people, working mostly from home. What you will do • Help with applications and appointments for maintenance requests. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with maintenance requests. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with quarterly visits in Atlanta. • Applications are reviewed on a rolling basis.
