Senior Case Manager - care planning (remote)

We are growing our team at Amberfield Community Partners and adding a Senior Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for volunteer shifts. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with clear deliverables from the first week. • Remote-friendly, with an occasional day on site in Seattle. • We reply to every applicant.
