Senior Case Manager - care planning (remote)

Silverline Juniper Civic Services Partners supports public agencies in Salt Lake City with budgeting, records and programmes, and we need a Senior Case Manager to lead our care planning. We follow procedures carefully and explain them clearly. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for permits. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with permits. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Salt Lake City. • Applications are reviewed on a rolling basis.
