Senior Case Manager - care planning (remote)

Lumen Ashton Civic Services Group provides shared administrative services to local governments around Colorado Springs, and we need a Senior Case Manager to lead our care planning. Public service means patience, paperwork and pride in the outcome. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for budget submissions. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with budget submissions. • Bilingual English and Spanish. Working arrangement • Contract for three months, with the possibility of becoming permanent. • Fully remote, with quarterly visits in Colorado Springs. • We will tell you what to expect at each step of the hiring process.
