Senior Case Manager - care planning (Raleigh, NC)

Lumen Birch Care Services Group is a social-care provider serving older adults and people with disabilities around Raleigh. We are hiring a Senior Case Manager to take on care planning. Teams support each other, especially on hard days. What you will do • Help with applications and appointments for licensing. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Raleigh, with some flexibility after the first few months. • We reply to every applicant.
