Senior Case Manager - care planning (Raleigh, NC)

Clearpath Orchard Care Services provides in-home and community support in Raleigh, and we need a Senior Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Raleigh, with some flexibility after the first few months. • We reply to every applicant.
