Senior Case Manager - care planning (Portland, OR)

Greenridge Fern Care Services Group is a social-care provider serving older adults and people with disabilities around Portland, and we need a Senior Case Manager to lead our care planning. Teams support each other, especially on hard days. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Portland. • Applications are reviewed on a rolling basis.
