Senior Case Manager - care planning (Pittsburgh, PA)

We are growing our team at Bluepeak Granite Community Partners Group and adding a Senior Case Manager whose main responsibility will be care planning. Flexibility and kindness are expected. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client intake. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Pittsburgh. • We reply to every applicant.
