Senior Case Manager - care planning (Pittsburgh, PA)

Rivermark Lantern Learning Group operates schools and learning programmes around Pittsburgh. We are hiring a Senior Case Manager to take on care planning. You would join a small organisation of about 30 people based in Pittsburgh. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for attendance records. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with attendance records. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Pittsburgh, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
