Senior Case Manager - care planning (Phoenix, AZ)

Bluepeak Bright Community Partners Group is a nonprofit in Phoenix supporting families with food, housing and job help. We are hiring a Senior Case Manager to take on care planning. Flexibility and kindness are expected. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client intake. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Phoenix, with some flexibility after the first few months. • We reply to every applicant.
