Senior Case Manager - care planning (Phoenix, AZ)

Northwind Harbor Care Services is looking for a Senior Case Manager. The job centres on care planning for visit scheduling, and it would suit someone who likes steady, practical work. The work is personal and so is the culture. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Phoenix, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
