Senior Case Manager - care planning (Philadelphia, PA)

Lumen Linden Civic Services is looking for a Senior Case Manager. The job centres on care planning for constituent requests, and it would suit someone who likes steady, practical work. You would join a large organisation of about 340 people based in Philadelphia. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for constituent requests. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with constituent requests. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • On site in Philadelphia. • Applications are reviewed on a rolling basis.
