Senior Case Manager - care planning (Philadelphia, PA)

Lumen Haven Early Learning is looking for a Senior Case Manager. The job centres on care planning for enrolment, and it would suit someone who likes steady, practical work. You would join a small organisation of about 152 people based in Philadelphia. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for enrolment. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Philadelphia. • We reply to every applicant.
